Jefferson Invites Area Social Workers & Case Managers to Learn about NJSave and Other Health-focused Services

Jefferson Health’s Community Outreach team invites area social workers and case managers who serve residents of low-income housing complexes, along with faith-and community-based organizations, to take advantage of its many health-focused services.

 

These include a free presentation about NJSave, a program that lets low-income older adults and those with disabilities apply for programs to save money on Medicare and Medicaid premiums, prescription costs, utilities, and other living expenses. In addition, Jefferson offers health screenings, vaccination clinics, language access workshops, and educational programs at no cost to the community.

Through a grant from the NJ Department of Human Services’ Division of Aging Services, Jefferson Health will offer assistance for those seeking to enroll in initiatives through NJSave. People who qualify and sign up for NJSave through a Jefferson program -– as well as finalize the process with a Jefferson representative -- may qualify for a gift card incentive.

 

“We are dedicated to addressing the needs of under-resourced community members throughout South Jersey,” says Aaron Chang, president of the Jefferson Health East Region. “This grant helps us further our mission of improving lives.”

 

Jefferson’s Community Outreach Team consists of healthcare and social workers who attend hundreds of community events per year, sharing health resources with diverse populations through educational presentations, pop-up vaccine clinics, mobile health screenings, and follow-up calls and care. The goal is to enroll at least 2,000 eligible individuals in NJSave.

 

Outreach team members, some of whom are multi-lingual, will provide educational workshops, enrollment events, help in obtaining documents, accessing and filling out the NJSave application, and following up, as needed.
