2024 Nomination Form

We are accepting nominations for the 2024 Gloucester Catholic Hall of Fame through December 1, 2023. Alumni, parents, faculty, staff, administrators, and friends may make nominations.  The following are the categories of recognition:

Arts & Academics - Alumni, teachers or friends of Gloucester Catholic High School who have distinguished themselves in the field of the arts or by outstanding achievements through scholarship, service, and leadership so as to reflect honor and distinction.

Athletics - Coaches, Players or Teams of Gloucester Catholic High School who have established distinctive records of achievement in athletics, which would make them deserving of recognition.

Special – Those alumni or friends of Gloucester Catholic High School whose actions and or achievements may not be categorized in any of the above, but are deserving of such recognition. These awards may be for humanitarian actions, community involvement, or other special contributions.

Please complete the requested information below. If you do not know all the information, fill out as much information as possible. We must be able to contact the nominee or their family in the event of their selection.

This form will be kept on file for a period of three years. If your nominee is not selected this year, selection could be take place in any of the remaining nomination cycles. You do not need to re-submit the nomination during this time period. All nominees on file are reviewed annually during their window of eligibility.
